If you have coverage with your insurance company for infertility services, here are some important questions to ask your insurance company when inquiring about your benefits are:
Does my insurance cover infertility benefits with a non-participating physician (i.e., a physician that is not a provider with my insurance)?
Does my insurance cover diagnostic evaluation for infertility (i.e., lab work, ultrasound, hysterosalpingogram (HSG), andrology services (Semen Analysis))?
Does my insurance cover treatment for infertility such as intrauterine insemination and/or in vitro fertilization?
- If so, does this include physician as well as hospital/surgery center?
- Are laboratory charges covered?
What is my deductible/co-payment for infertility related charges?
Does my insurance cover oral or injectable medications (i.e., Clomid, Follistim, Repronex, Bravelle, Lupron, Human Chorionic Gonadotropin (HCG), Antagon)?
Does my insurance require use of a specific contracted pharmacy?
Does my insurance require use of a specific contracted laboratory?
